• Independent Living Skills Trainer would visit prospective clients to assess their functional living skills ability and work with them to develop a service plan.
• ILST staff would educate HCSS providers on how to follow the individual service plan, working with the individual to train them in the areas of basic living skills to promote their independence.
• ILST would monitor the progress of the individual based on HCSS reports of milestones achieved.
